Sometimes it's reused CAT5e that can't handle the bandwidth of modern 4K video. Sometimes the installer kinked wires during the run and degraded the data signal over time. Sometimes the connections weren't terminated properly, which causes intermittent power failures over PoE. The failure modes vary but the root cause rarely does.
